POSITION
Summary:
The DCW understands and demonstrates the importance, role and scope of practice of the DCW profession and their rights and responsibilities as a DCW to master all needed competencies, uphold high professional and ethical standards, abide by agency, legal and Health Insurance Portability and Accountability Act of 1996 (HIPAA) rules, support care coordination, participate in teams and organizational life, and protect participant rights across all work settings.
The DCW works in a professional and ethical manner, maintaining confidentiality and respecting individual and family rights.
The DCW understands that person-centered support and services are based on building trusting, safe relationships, uses person-centered practices, assists individuals/families to make choices and plan goals, and provides support to help individuals achieve goals based on their values, preferences, beliefs, cultures, etc.
The DCW builds trust and productive relationships with individuals, family members, co-workers and others through respectful and clear verbal and written communications.
The DCW closely monitors an individual's physical and emotional health, gathers information about the individual, and communicates observations to the support and services team to guide services.
The DCW identifies risks and behaviors that can lead to a crisis and uses effective strategies to prevent or intervene in the crisis in collaboration with others.
The DCW is attentive to signs of abuse, neglect, or financial exploitation and follows procedures for mandated reports to protect an individual from such harm.
The DCW helps people to avoid unsafe situations and uses appropriate procedures to assure safety during emergency situations.
The DCW provides advocacy and empowers and assists individuals to advocate for what they need and value in life.
The DCW plays a vital role in supporting individuals to achieve and maintain good physical, mental, social, and emotional health essential to their well-being.
The DCW helps individuals to manage personal and household tasks that are necessary on a day-to-day basis to pursue an independent, community-based lifestyle.
The DCW helps individuals to be a part of the community through valued roles and relationships and assists with major transitions that occur in community life.
The DCW respects cultural differences and provides services and supports that honor individual preferences.
The DCW is aware of the requirements for documentation in their organization and is able to manage these requirements efficiently.
Qualifications:
Provide (3) professional and or personal references supporting the applicant's good moral character and their ability to treat persons with dignity and respect, problem solve in stressful situations, and follow written and oral instructions that are related to the care and supervision of the person(s) served including adults identified as vulnerable.
Provide verification of experience, education, and or training.
Examples:
Diploma, GED, Educational transcripts, Training Records, Skill Assessment Results.
Two years experience working with persons with developmental disabilities and or mental illness or substance abuse disorders is preferred.
Lived experiences with behavioral health issues are desired.
Provide evidence of understanding written and verbal instructions and possess the ability to accurately and effectively communicate both verbally and in writing.
As part of the conditional employment the applicant is required to pass and maintain certification in the Department of Community Health training requirements and the training requirements set forth by the Contracting Community Mental Health Authorities and the company.
Criminal Clearance; The AFC Facility Licensing Act prohibits AFC facilities from employing or independently contracting with an individual with certain criminal conviction histories who regularly has direct access to or provides direct services to residents.
Applicants provide evidence of age; 18 years of age or older.
Possession of a current valid Operators' license and provide evidence of a safe driving record to be able to transport.
Meet the LARA Medical Clearance Requirements, including TB clearance.
Must be able to read medication labels, and physician orders to properly set up and administer medications.
Ability to safely lift up to 50lbs.
Direct Care Staff must be physically able to do lifting and or transferring ofpersons who are physically impaired and need assistance.
Must be able to safely lift by keeping the back aligned, bending at the knees, maintaining the elbows close to their side as applicable, be able to step and turn, reach upward, and also have the ability to occasionally remain on feet for long periods of time.
Must be able to handle and use cleaning products properly; according to the manufacturer instructions.
ESSENTIAL JOB FUNCTIONS:
Demonstrate a positive and willing attitude, evidenced through verbal and nonverbal communication.

Adhere to the company personnel policies and procedures including the Code of Ethics and the Home and Community Based Service Rules.
Awake at all times.
Set up, and administer medications according to the physician orders.
Accurately and descriptively document measurable data that corresponds with the treatment plan goals and objectives for each resident prior to the end of the scheduled shift.
Accurately complete and promptly submit incident reports to the Administrator within 24 hours of incident.
Notify the Manager of all incident reports prior to the end of the shift.
Report suspected abuse and or neglect to the proper authorities.
Accurately inventory and administer medication according to the physician's order.
Maintain a safe and clean living and working environment for all persons.
Routine property maintenance and housekeeping including general cleaning, dishes, sweeping, mopping,vacuuming, dusting, restroom cleaning, trash removal, laundry, window washing, clearing walkways of snow and ice, etc.
Promptly report all home and vehicle maintenance needs on the maintenance report form and to the manager.
Coordinate and participate with the residents in leisure activities in the home and out in the community.
Know, and implement treatment plan objectives with the residents.
Review and remain up to date with policy and contractual requirements regularly.
Food preparation according to established guidelines.
Maintain confidentiality.
Compliance with Resident Rights prescribed by the Office of Recipient Rights, Michigan Mental Health Code, MDHHS, and HCBS Rules.
Adhere to the Mandated Reporter Requirements Attend and participate during scheduled staff meetings and or inservice or training.
In case of an emergency or staffing shortage, the DSP must ensure he or she stays on to work the next scheduled shift until the appropriate number of personnel are in place.
