PACE is an alternative to nursing home care and - through an interdisciplinary care team (IDT) of physicians, nurse practitioners, nurses, social workers, therapists, van drivers, and aides - coordinates and provides preventive, primary, acute, and long-term care services, so older individuals can continue living in the community.

This includes evaluating the participant's nutrition status, developing and implementing nutrition care plans, assessing nutrient requirements and evaluating effectiveness of nutrition care.
Responsible for communicating nutrition status to the physicians and Interdisciplinary Team (IDT) members.
Key Responsibilities Assess, develop, and implement care plans.
Monitor meals in the day center and in the home.
Manage therapeutic and special diets.
Monitor key clinical indicators of nutrition status.
Manage Nutrition Support.
Supervisory Responsibilities This position has no supervisory responsibilities.
Qualifications Bachelor's Degree from an Academy of Nutrition and Dietetics accredited institution with a major in food and nutrition or dietetics; Master's Degree preferred.
Completion of an accredited Dietetic internship with a minimum of 1,200 hours of supervised practice hours.
Minimum of one year of experience working with the frail, elderly, or long-term care population.
Work Environment Has visible presence while working in the center and is subject to frequent interruptions.
Communicates with staff, participants, caregivers, and other organizations, as necessary.
Works beyond normal working hours and on weekends and holidays, as necessary.
